ORDER SUMMARY:

1. Daniel Rothweiler (“Rothweiler”), as an Unemployment Compensation Claims Examiner for the Pennsylvania Department of Labor and Industry, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act, 65 Pa.C.S. § 1104(a), when he failed to timely file a Statement of Financial Interests for calendar year 2016 with the Pennsylvania Department of Labor and Industry.

2. This Commission hereby levies one maximum civil penalty against Rothweiler at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2016 remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00). Rothweiler is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. Failure to comply with Paragraph 2 of this Order will result in the initiation of an appropriate enforcement action.

Order No. 598-S (Alexander)
Unemployment Compensation Tax Technician
Pennsylvania Department of Labor and Industry

ORDER SUMMARY:

1. Helen Alexander (“Alexander”), as an Unemployment Compensation Tax Technician for the Pennsylvania Department of Labor and Industry, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act, 65 Pa.C.S. § 1104(a), when she failed to timely file a Statement of Financial Interests for calendar year 2016 with the Pennsylvania Department of Labor and Industry.

2. Based upon the totality of the circumstances in this case, this Commission hereby levies one civil penalty against Alexander in the total amount of One Hundred and Fifty Dollars ($150.00) for the delinquent Statement of Financial Interests for calendar year 2016. Alexander is ordered to pay said civil penalty in the total amount of $150.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. Failure to comply with Paragraph 2 of this Order will result in the initiation of an appropriate enforcement action.

Order No. 599-S (Brooks)
Constable/Deputy Constable
Pittsburgh, PA

ORDER SUMMARY:

1. Roy Brooks (“Brooks”), as a Constable/Deputy Constable, failed to timely file a Statement of Financial Interests for the 2016 calendar year in compliance with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a).

2. Per the Consent Agreement of the parties, we hereby levy one civil penalty against Brooks in the total amount of $250.00 for Brooks’s delinquent Statement of Financial Interests for calendar year 2016.

3. To the extent he has not already done so, Brooks is ordered to make payment of the aforesaid civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding to this Commission a certified check or money order in the amount of $250.00 made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

4. To the extent he has not already done so, Brooks is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2016 with this Commission by no later than the thirtieth (30th) day after the mailing date of this Order.

5. Failure to comply with Paragraph 3 or 4 of this Order will result in the initiation of an appropriate enforcement action.

ORDER SUMMARY:

1. Pamela Yuhaniak (“Yuhaniak”), as an Income Maintenance Caseworker for the Pennsylvania Department of Human Services, failed to timely file a Statement of Financial Interests for the 2016 calendar year in compliance with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a).

2. Per the Consent Agreement of the parties, we hereby levy one civil penalty against Yuhaniak in the total amount of $250.00 for Yuhaniak’s delinquent Statement of Financial Interests for calendar year 2016.

3. To the extent she has not already done so, Yuhaniak is ordered to make payment of the aforesaid civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding to this Commission a certified check or money order in the amount of $250.00 made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

4. To the extent she has not already done so, Yuhaniak is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2016 with the Pennsylvania Department of Human Services by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to this Commission for compliance verification purposes.

5. Failure to comply with Paragraph 3 or 4 of this Order will result in the initiation of an appropriate enforcement action.

Order No. 601-S (Salisbury)
Constable/Deputy Constable
Lancaster, PA

ORDER SUMMARY:

1. Karl Salisbury (“Salisbury”), as a Constable/Deputy Constable, failed to timely file a Statement of Financial Interests for the 2016 calendar year in compliance with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a).

2. Per the Consent Agreement of the parties, we hereby levy one civil penalty against Salisbury in the total amount of $250.00 for Salisbury’s delinquent Statement of Financial Interests for calendar year 2016.

3. To the extent he has not already done so, Salisbury is ordered to make payment of the aforesaid civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding to this Commission a certified check or money order in the amount of $250.00 made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

4. To the extent he has not already done so, Salisbury is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2016 with this Commission by no later than the thirtieth (30th) day after the mailing date of this Order.

5. Failure to comply with Paragraph 3 or 4 of this Order will result in the initiation of an appropriate enforcement action.
