ORDER SUMMARY:

1. Ketaurah Caldwell (“Caldwell”), as a former Income Maintenance Caseworker for the Pennsylvania Department of Human Services, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act, 65 Pa.C.S. § 1104(a), when she failed to timely file a Statement of Financial Interests for calendar year 2016 with the Pennsylvania Department of Human Services.   
2. This Commission hereby levies one maximum civil penalty against Caldwell at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2016 remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Caldwell is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.
3. Failure to comply with Paragraph 2 of this Order will result in the initiation of an appropriate enforcement action.    


Order No. 629-S (Booker)
Constable/Deputy Constable

ORDER SUMMARY:

1. Barry Booker (“Booker”), as a Constable/Deputy Constable, failed to timely file a Statement of Financial Interests for the 2017 calendar year in compliance with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a).
2. Per the Consent Agreement of the parties, we hereby levy one civil penalty against Booker in the total amount of $250.00 for Booker’s delinquent Statement of Financial Interests for calendar year 2017.
3. To the extent he has not already done so, Booker is ordered to make payment of the aforesaid civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding to this Commission a certified check or money order in the amount of $250.00 made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.
4. To the extent he has not already done so, Booker is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2017 with this Commission by no later than the thirtieth (30th) day after the mailing date of this Order.
5. Failure to comply with Paragraph 3 or 4 of this Order will result in the initiation of an appropriate enforcement action.


Order No. 630-S (Kelly)
Constable/Deputy Constable

ORDER SUMMARY:

1. James Kelly (“Kelly”), as a Constable/Deputy Constable, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a), when he failed to timely file a Statement of Financial Interests for calendar year 2017 with the Pennsylvania State Ethics Commission.
2. This Commission hereby levies one maximum civil penalty against Kelly at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2017 remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Kelly is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.
3. Failure to comply with Paragraph 2 of this Order will result in the initiation of an appropriate enforcement action.

ORDER SUMMARY:

1. Teresa Randleman (“Randleman”), as a former Equal Opportunity Specialist 3 for the Pennsylvania Department of Human Services, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a), when she failed to file a Statement of Financial Interests for calendar year 2016 with the Pennsylvania Department of Human Services.
2. This Commission hereby levies one maximum civil penalty against Randleman at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2016 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Randleman is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.
3. Randleman is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2016 with the Pennsylvania Department of Human Services by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes.
4. Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.
