ORDER SUMMARY:

1. Samuel L. Bryant (“Bryant”), as a Constable/Deputy Constable, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a), when he failed to file a Statement of Financial Interests for calendar year 2019 with the Pennsylvania State Ethics Commission.

2. This Commission hereby levies one maximum civil penalty against Bryant at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2019 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Bryant is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. Bryant is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2019 with the Pennsylvania State Ethics Commission by no later than the thirtieth (30th) day after the mailing date of this Order.

4. Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.

1. Michael Cornell (“Cornell”), as a Constable/Deputy Constable, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a), when he failed to file a Statement of Financial Interests for calendar year 2019 with the Pennsylvania State Ethics Commission.

2. This Commission hereby levies one maximum civil penalty against Cornell at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2019 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Cornell is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

2. Angelina Ellis (“Ellis”), as a Health Facility Quality Examiner for the Pennsylvania Department of Health, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a), when she failed to file a Statement of Financial Interests for calendar year 2019 with the Pennsylvania Department of Health.

2. This Commission hereby levies one maximum civil penalty against Ellis at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2019 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Ellis is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. 2. This Commission hereby levies one maximum civil penalty against Bertolini at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2019 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Bertolini is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. Bertolini is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2019 with the Pennsylvania State Ethics Commission by no later than the thirtieth (30th) day after the mailing date of this Order.

Jarrett Seigle (“Seigle”), as a Workforce Development Analyst II for the Pennsylvania Department of Labor and Industry, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a), when he failed to file a Statement of Financial Interests for calendar year 2019 with the Pennsylvania Department of Labor and Industry.

2. This Commission hereby levies one maximum civil penalty against Seigle at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2019 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Seigle is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. 2. This Commission hereby levies one maximum civil penalty against Sinkovich at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2019 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Sinkovich is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. Sinkovich is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2019 with the Pennsylvania  Department of Environmental Protection by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes.

4. Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.

William Dunleavy (“Dunleavy”), as an Income Maintenance Caseworker for the Pennsylvania Department of Human Services, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a), when he failed to file a Statement of Financial Interests for calendar year 2019 with the Pennsylvania Department of Human Services.

2. This Commission hereby levies one maximum civil penalty against Dunleavy at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2019 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Dunleavy is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. Dunleavy is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2019 with the Pennsylvania Department of Human Services by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes.

ORDER SUMMARY:


1. Elizabeth Crowley (“Crowley”), as the Secretary and Treasurer for Mount Jewett Borough, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a), when she failed to file Statements of Financial Interests for calendar years 2017 and 2018 with Mount Jewett Borough.

2. This Commission hereby levies two maximum civil penalties against Crowley, each at the rate of Twenty-Five Dollars ($25.00) per day, for each day her Statements of Financial Interests for calendar years 2017 and 2018 have remained delinquent, for total civil penalty liability of Five Hundred Dollars ($500.00).  Crowley is ordered to pay said civil penalties in the total amount of $500.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. Crowley is ordered to file complete and accurate Statements of Financial Interests for calendar years 2017 and 2018 with Mount Jewett Borough by no later than the thirtieth (30th) day after the mailing date of this Order, with a copy of each such filing forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes.

4. Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.
 

Order No. 694-S (Wilson)

ORDER SUMMARY:


1. William Wilson (“Wilson”), as an Unemployment Compensation Claims Examiner for the Pennsylvania Department of Labor and Industry, failed to timely file a Statement of Financial Interests for the 2019 calendar year in compliance with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a).  

2. Per the Consent Agreement of the parties, we hereby levy one civil penalty against Wilson in the total amount of $150.00 for Wilson’s delinquent Statement of Financial Interests for calendar year 2019.

3. To the extent he has not already done so, Wilson is ordered to make payment of the aforesaid civil penalty in the total amount of $150.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding to this Commission a certified check or money order in the amount of $150.00 made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury. 

4. To the extent he has not already done so, Wilson is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2019 with the Pennsylvania Department of Labor and Industry by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to this Commission for compliance verification purposes.

5. Failure to comply with Paragraph 3 or 4 of this Order will result in the initiation of an appropriate enforcement action.

2. This Commission hereby levies one maximum civil penalty against Parks at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2019 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Parks is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. 3. 2. This Commission hereby levies one maximum civil penalty against Gonzalez-Okert at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2019 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Gonzalez-Okert is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. Gonzalez-Okert is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2019 with the Pennsylvania Department of Banking and Securities by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes.

4. Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.
