1. Steven Swalm (“Swalm”), as an Assistant Director of Revenue for Cash Flow and Debt for the Executive Offices of the Governor, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a), when he failed to file a Statement of Financial Interests for calendar year 2016 with the Executive Offices of the Governor.
2. This Commission hereby levies one maximum civil penalty against Swalm at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2016 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Swalm is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.
3. Swalm is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2016 with the Executive Offices of the Governor by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the State Ethics Commission for compliance verification purposes.
4. Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.

 

1. Christina Faye (“Faye”), as an Income Maintenance Caseworker for the Pennsylvania Department of Human Services, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a), when she failed to file a Statement of Financial Interests for calendar year 2016 with the Pennsylvania Department of Human Services.
2. This Commission hereby levies one maximum civil penalty against Faye at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2016 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Faye is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.
3. Faye is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2016 with the Pennsylvania Department of Human Services by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes.
4. Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.

 

1. Susan Hartz (“Hartz”), as an Income Maintenance Caseworker for the Pennsylvania Department of Human Services, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a), when she failed to file a Statement of Financial Interests for calendar year 2016 with the Pennsylvania Department of Human Services.
2. This Commission hereby levies one maximum civil penalty against Hartz at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2016 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Hartz is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

2. This Commission hereby levies one maximum civil penalty against Rawlins at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2016 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Rawlins is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

2. This Commission hereby levies one maximum civil penalty against Sullivan at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2016 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Sullivan is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

2. This Commission hereby levies one maximum civil penalty against Christopher at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2015 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Christopher is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

1. Terrence Crumby (“Crumby”), as a Constable/Deputy Constable, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a), when he failed to file Statements of Financial Interests for calendar years 2013, 2014, 2015, and 2016 with the Pennsylvania State Ethics Commission.
2. This Commission hereby levies four maximum civil penalties against Crumby, each at the rate of Twenty-Five Dollars ($25.00) per day, for each day his Statements of Financial Interests for calendar years 2013, 2014, 2015, and 2016 have remained delinquent, for total civil penalty liability of One Thousand Dollars ($1,000.00).  Crumby is ordered to pay said civil penalties in the total amount of $1,000.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.
3. Crumby is ordered to file complete and accurate Statements of Financial Interests for calendar years 2013, 2014, 2015, and 2016 with the Pennsylvania State Ethics Commission by no later than the thirtieth (30th) day after the mailing date of this Order.
4. Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.

 

1. Matthew Toso (“Toso”), as a Constable/Deputy Constable, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a), when he failed to file a Statement of Financial Interests for calendar year 2016 with the Pennsylvania State Ethics Commission.
2. This Commission hereby levies one maximum civil penalty against Toso at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2016 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Toso is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.
3. Toso is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2016 with the Pennsylvania State Ethics Commission by no later than the thirtieth (30th) day after the mailing date of this Order.

2. This Commission hereby levies one maximum civil penalty against Stanec at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2016 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Stanec is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

2. This Commission hereby levies one maximum civil penalty against Rowles at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2016 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Rowles is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.
