ORDER SUMMARY:

1. Mark Jeannette (“Jeannette”), as an Administrative Officer 2 for the Pennsylvania Department of Community and Economic Development, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act, 65 Pa.C.S. § 1104(a), when he failed to timely file a Statement of Financial Interests for calendar year 2017 with the Pennsylvania Department of Community and Economic Development. 
2. This Commission hereby levies one maximum civil penalty against Jeannette at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2017 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Jeannette is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.
3. Jeannette is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2017 with the Pennsylvania Department of Community and Economic Development by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes.
4. Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.


Order No. 664-S (Mingo)
Insurance Company Examiner 2
Pennsylvania Department of Insurance 
Harrisburg, PA  17120

ORDER SUMMARY:

1. Bernard Mingo (“Mingo”), as an Insurance Company Examiner 2 for the Pennsylvania Department of Insurance, failed to timely file a Statement of Financial Interests for the 2018 calendar year in compliance with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a). 
2. Per the Consent Agreement of the parties, we hereby levy one civil penalty against Mingo in the total amount of $150.00 for Mingo’s delinquent Statement of Financial Interests for calendar year 2018.
3. Per the Consent Agreement of the parties, Mingo is ordered to make payment of the aforesaid civil penalty in the total amount of $150.00--by way of monthly payments of no less than $25.00 each for a period of six months--as follows: (1) each such payment shall be made by forwarding to this Commission a certified check or money order in the amount of no less than $25.00 payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury; and (2) the first such payment shall be due by no later than the thirtieth (30th) day after the mailing date of this Order. 
4. To the extent he has not already done so, Mingo is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2018 with the Pennsylvania Department of Insurance by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to this Commission for compliance verification purposes.
5. Failure to comply with Paragraph 3 or 4 of this Order will result in the initiation of an appropriate enforcement action.


Order No. 665-S (Kindt)

Pennsylvania State Apprenticeship and Training Council

ORDER SUMMARY:

1. Barry Kindt (“Kindt”), as a Member of the Pennsylvania State Apprenticeship and Training Council, failed to timely file Statements of Financial Interests for the 2014, 2015, 2016, 2017, and 2018 calendar years in compliance with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a). 
2. Per the Consent Agreement of the parties, we hereby levy civil penalties in the total amount of $625.00 against Kindt for Kindt’s delinquent Statements of Financial Interests for calendar years 2014, 2015, 2016, 2017, and 2018.
3. To the extent he has not already done so, Kindt is ordered to make payment of the aforesaid civil penalties in the total amount of $625.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding to this Commission a certified check or money order in the amount of $625.00 made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.
4. Failure to comply with Paragraph 3 of this Order will result in the initiation of an appropriate enforcement action.
