ORDER SUMMARY:


1. Dennis Harrsch (“Harrsch”), as a Staff Physician 2 for the Pennsylvania Department of Human Services, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a), when he failed to file a Statement of Financial Interests for calendar year 2018 with the Pennsylvania Department of Human Services.

2. This Commission hereby levies one maximum civil penalty against Harrsch at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2018 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Harrsch is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. Harrsch is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2018 with the Pennsylvania Department of Human Services by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes.

4. Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.


Order No. 679-S (Still) 
High School Principal
Susquehanna Township School District
Harrisburg, PA  17109

ORDER SUMMARY:


1. Keith Still (“Still”), as a High School Principal for the Susquehanna Township School District, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a), when he failed to file a Statement of Financial Interests for calendar year 2018 with the Susquehanna Township School District.

2. This Commission hereby levies one maximum civil penalty against Still at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2018 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Still is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. Still is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2018 with the Susquehanna Township School District by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes.

Income Maintenance Caseworker

ORDER SUMMARY:


1. Akilah A. Ingram (“Ingram”), as an Income Maintenance Caseworker for the Pennsylvania Department of Human Services, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a), when Ingram failed to file a Statement of Financial Interests for calendar year 2018 with the Pennsylvania Department of Human Services.

2. This Commission hereby levies one maximum civil penalty against Ingram at the rate of Twenty-Five Dollars ($25.00) per day for each day Ingram’s Statement of Financial Interests for calendar year 2018 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Ingram is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. Ingram is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2018 with the Pennsylvania Department of Human Services by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes.

ORDER SUMMARY:


1. Shariyf B. Grevious (“Grevious”), as an Income Maintenance Caseworker for the Pennsylvania Department of Human Services, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a), when he failed to file a Statement of Financial Interests for calendar year 2018 with the Pennsylvania Department of Human Services.

2. This Commission hereby levies one maximum civil penalty against Grevious at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2018 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Grevious is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. Grevious is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2018 with the Pennsylvania Department of Human Services by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes.

ORDER SUMMARY:


1. Thomas Gustovich (“Gustovich”), as a School Director for the Bethlehem-Center School District, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a), when he failed to file Statements of Financial Interests for calendar years 2015, 2016, and 2017 with the Bethlehem-Center School District.

2. This Commission hereby levies three maximum civil penalties against Gustovich, each at the rate of Twenty-Five Dollars ($25.00) per day, for each day his Statements of Financial Interests for calendar years 2015, 2016, and 2017 have remained delinquent, for total civil penalty liability of Seven Hundred and Fifty Dollars ($750.00).  Gustovich is ordered to pay said civil penalties in the total amount of $750.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. Gustovich is ordered to file complete and accurate Statements of Financial Interests for calendar years 2015, 2016, and 2017 with the Bethlehem-Center School District by no later than the thirtieth (30th) day after the mailing date of this Order, with a copy of each such filing forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes.

4. Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.


Order No. 683-S (Parker-Berhannan)

Executive Offices of the Governor
Philadelphia, PA  19144

ORDER SUMMARY:


1. Lataya Parker-Berhannan (“Parker-Berhannan”), as a Human Relations Representative 1 for the Executive Offices of the Governor, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act, 65 Pa.C.S. § 1104(a), when she failed to timely file a Statement of Financial Interests for calendar year 2018 with the Executive Offices of the Governor. 

2. This Commission hereby levies one maximum civil penalty against Parker-Berhannan at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2018 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00). Parker-Berhannan is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. Failure to comply with Paragraph 2 of this Order will result in the initiation of an appropriate enforcement action.
