1. Manuel Villeneuve (“Villeneuve”), as an Income Maintenance Caseworker for the Pennsylvania Department of Human Services, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a), when he failed to file Statements of Financial Interests for calendar years 2018 and 2019 with the Pennsylvania Department of Human Services.

2. This Commission hereby levies two maximum civil penalties against Villeneuve, each at the rate of Twenty-Five Dollars ($25.00) per day, for each day his Statements of Financial Interests for calendar years 2018 and 2019 have remained delinquent, for total civil penalty liability of Five Hundred Dollars ($500.00). Villeneuve is ordered to pay said civil penalties in the total amount of $500.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. Villeneuve is ordered to file complete and accurate Statements of Financial Interests for calendar years 2018 and 2019 with the Pennsylvania Department of Human Services by no later than the thirtieth (30th) day after the mailing date of this Order, with a copy of each such filing forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes.

4. Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.

This Commission hereby levies one maximum civil penalty against Williams at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2019 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00). 2. This Commission hereby levies one maximum civil penalty against Hubert at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2019 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00). Hubert is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. 2. This Commission hereby levies two maximum civil penalties against Tejada, each at the rate of Twenty-Five Dollars ($25.00) per day, for each day her Statements of Financial Interests for calendar years 2018 and 2019 have remained delinquent, for total civil penalty liability of Five Hundred Dollars ($500.00). Tejada is ordered to pay said civil penalties in the total amount of $500.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

3. 2. This Commission hereby levies three maximum civil penalties against Laws, each at the rate of Twenty-Five Dollars ($25.00) per day, for each day his Statements of Financial Interests for calendar years 2017, 2018 and 2019 have remained delinquent, for total civil penalty liability of Seven Hundred and Fifty Dollars ($750.00). Laws is ordered to pay said civil penalties in the total amount of $750.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

1. Albert F. Cafazzo (“Cafazzo”), as a Constable/Deputy Constable, failed to timely file a Statement of Financial Interests for the 2020 calendar year in compliance with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act”), 65 Pa.C.S. § 1104(a).

2. Per the Consent Agreement of the parties, we hereby levy one civil penalty against Cafazzo in the total amount of $250.00 for Cafazzo’s delinquent Statement of Financial Interests for calendar year 2020.

3. To the extent he has not already done so, Cafazzo is ordered to make payment of the aforesaid civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by forwarding to this Commission a certified check or money order in the amount of $250.00 made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

4. To the extent he has not already done so, Cafazzo is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2020 with this Commission by no later than the thirtieth (30th) day after the mailing date of this Order.
